If you take a look at section 4 in the FAQ, I've added how to do it
in 2.2 (which is so much easier than any of the earlier versions of
SSH2).

Gregor did a great write-up on how to get SSH2 and SSH1 working with
hostbased authentication, and that's in section 9.4.5.

I don't know how frequently the mirrors update, but mine is usually the
first to post changes: http://www.tigerlair.com/ssh/faq. I'm not sure
how frequently Steve makes the changes at 
http://www.employees.org/~satch/ssh/faq.

There are 6 other sites that mirror the FAQ.
