Look in the source tree of openSSH in the contrib directory and you'll
find the PAM stuff for redhat. Drop the provided file in the
/etc/pam.d directory as "sshd" and if it is a PAM related problem it
should clear it up. If memory serves, the errors you listed are in
fact because sshd has no presence in /etc/pam.d
