Commit cebc2de44d3bce53e46476e774126c298ca2c8a9 ("dm space map metadata:
fix refcount decrement below 0 which caused corruption") needs to be
backported to stable 3.12+.
I marked that commit header for stable 3.13+ but at that time I didn't
realize commit 5b564d80 ("dm space map: disallow decrementing a
reference count below zero") was tagged for 3.12+ -- so stable 3.12 is
exposed without this fix, as was confirmed via this BZ:
