Thanks for the feedback Ivar, much appreciated.
I opened a SR via the My Oracle support site (3-2984566821) and with the
advice of the engineer we inserted just the -D option and that resolved
the issue for us. I am still waiting for my clients to confirm the
issue is resolved but I can no longer reproduce it.
The details of what was done are below from Oracle (Carlos Arellano)

/etc/opt/SUNWkio/sessions/vda/vda-uttsc
# Now run uttsc with the given arguments
# Forward uttsc output to syslog
# Use a marker file, to detect a non-zero exit code
rm -f $theUTTSCFailure

You want to add -D right after $theUTTSCExec

I had I think the same issue with SRS:
My solution was inserting the uttsc options: -N off -U 0 -D
