I recently purchased a Sapphire Radeon 7000. This was a PC card that was
flashed with a Mac BIOS. I have tried everything I can think of but I can't
get it to work. I can get a refund or exchange but before I do I wanted to
make sure I wasn't overlooking something.

I downloaded and tried all three sets of drivers ATI provides on its web
site with OS 9.2.2 and got nothing.
I booted from an OS-9.1 installer disk and got nothing.

When I use my old video card (Voodoo5) in another slot so I can see what's
going on, the ATI Config Menu says "cannot find an ATI display product"
Apple System Profiler can see a card in the slot but shows very little info:
"PCI Slot (Display)
Card type: Display
Card name: pci10002, 5159
Card model: Not available
Card ROM#: Not available
Card Revision: 0
Card Vendor ID: 1002"

I only have one monitor so I have to switch it back and forth between the
two cards to test the Radeon - don't know if that's an issue or not.

Any ideas or is this card dead or not flashed correctly?

Based on the info you have provided it appears to me that the card is not flashed to Mac at all or was a bad flash. It should work in OS 9.x without any drivers other then the ones included with the Mac OS. It will however be running slower with slow screen redraws etc.
I can provide more info if you would like to try flashing your self . You may well need a PC and need to know how to run Dos booted from a floppy disk. Most Mac users can not do this you have been warned ;-) Will S
