Originally reported by a Firefox 17 user, I was having the exact same problem with "2.14" - cookie permissions not being saved from one session to the next.
Using "sqlitebrowser", I deleted a number of existing permissions, reducing the size of the "permissions.sqlite" file from 623K to 26K. Everything now seems to be working normally.
The OP postulates (correctly, I think) that some change from F16 to F17 (and therefore SM 2.13.2 to SM 2.14) imposed a limit (deliberately or otherwise) on the size of the "permissions" database. -JW
