Are you using 1 to 1 NAT with an IP other than the WAN IP? I know on the PIX you must create a static 1 to 1 mapping in order to get PPTP to work. Haven't had a chance to test that with PFSense.

 
 
Yes I am using 1:1 for the PPTP port, I've tried with and without it.
