If you know the address and irq assigned to it, you
should be able to do one of the following:

modprobe eepro io=address irq=yourirq  mem=address

or you can put the info in conf.modules and just do
a "modprobe eepro"

eepro.o is the module used for the Intel EtherExpressPro.

If you have a dualboot with windows, you can check
what settings windows is using for the card and
use them for your settings.
