ikev2: cleanup process_v2_child_ix()'s caller
Drop md->st; ...(); md->st=NULL sequences. ...() isn't using md->st.
Use struct child_sa more; assume it isn't NULL.
Add more notes about how the existing ST might be an IKE SA and/or
a CHILD SA.
