It's remotely possible this "error" is actually a bug with the flash plugin 
itself... I may be totally off on this, but the non-descript 
"NS_ERROR_FAILURE" part is very suspiciously like what is seen in certain 
browsers as a result of the flash bug mentioned below.  That same error text 
shows up in firefox's logs with every load of a SWF in certain scenarios 
(like inside iframes, for instance), even though all the flash and page 
seems to work just fine. so, given that and its similarity to your symptoms, 
i figured I'd just throw it out there just in case.
