For FF to work with 100% sizing, you need the CSS as described with FA #1. If you don't most likely the minimum size restrictions for the ExpressInstall dialog are not being met, and it is never displayed.
You shouldn't need any modifications to the FAQ#1 example code in order to display ExpressInstall.
