'Bad annotaton destination' aside, using verbose switches does give further 
info.
Namely,

'This file is too complex to render - SWF only supports 65536 shapes at once'

.. at which point the conversion bails out.

Which I suppose is your answer! ;o)

If you must convert, then probably wise better to tweak the existing pdf first.
