Great, I would favor $_Mailer because of laziness, but I will live with
$mailerService, and I definitely agree it's more clear, although I'm
sure many people will be confused with the notion of services at first,
so either way it has to be well documented.

I tend to prefer the Service suffix as this more explicit. The _ as a prefix has already a meaning in many PHP applications (protected variables).
