This question is best for the symfony-users group, please ask future
questions there. It looks like you need to enable annotations in your
tests. Something like this to get your form object:
$validator = Validation::createValidatorBuilder()
->enableAnnotationMapping()
->getValidator();
$factory = Forms::createFormFactoryBuilder()
->addExtension(new ValidatorExtension($validator))
->getFormFactory();
$form = $factory->create( <Your FormType Here>
