please help me i make this code

in order to send mail to perssone with i revover it from form i
work in the backend but i have this error
500 | Internal Server Error | sfRenderException
The template "createSuccess.php" does not exist or is unreadable in "".
im blockend and i must resolve this problem
i will be grateful
