It is seems ,that it is better if the Fault Mediator only create a SOAPFault
Element and then replace the current SOAP Envelope Body with it without
changing SOAP Headers and The logic need to Decide whether this Fault need to
sent back to the client or to the back-end service,need to remove from the
Fault Mediator. Then After finishing message mediation in the Fault Mediator,It
is possible to change headers(e.g : using header mediator) So that Fault can
send back to the client or to the back-end service.
