speaking of the Italian situation, zebra markings on traffic signal controlled crossings are customary. These are crossing=traffic_signals in OpenStreetMap. Then there are zebra crossings (i.e. zebra markings and no signals), where in Italy an additional pedestrian crossing sign is required _only_ if the crossing is distant to a road crossing/intersection.
