That's just because Potlatch doesn't close the changeset unless you manually close it. The server will shut down open changesets after 1 hour in inactivity. If you shut down Potlatch, then reopen it it'll start a new changeset. The 412 can happen any time you try to revert something which has been edited since... if he was still editing it's more than likely -- you might need to consider both changesets together to do a proper revert (assuming Frederick's tool lets you do that.. not sure it does).
