I lost mine, as well so will await the response of the folks who can help with 
this.  Mine are in my
iPhone 6 as well as on my desktop in CSV format.  What a pain!  I requested 
help from the Microsoft
Disability Team and they did not populate my address book fields but did put 
the stuff on my
desktop.  +
