My observations are subjective at best, but I think that Microsoft Office products somehow affect focusing and sometimes even how the cursor works in other applications. If you have the problem in NotePad again, it would be interesting to know if it clears up without closing and opening the file if you close all Microsoft programs. Be sure that there isn't a Microsoft OneDrive loggin prompt open.
Since I have been compelled to use JAWS more, I have noticed that when it gets confused, it has a "Lost Focus" message which seems to happen most frequently when Microsoft Office products are running, particularly Outlook. I mention that here because I think screen readers may be encountering some similar issues but handling them differently.
