Hi Mike, I am inclined to suggest that you somehow reset the form. The easiest method may be to run down to the bottom of the page and see if you can find a reset form button. Alternatively, try to return to the page From a clean link, that is, not from a bookmark that you may have saved. This latter suggestion might be a bit more complicated, but the idea is to get the website to serve you up a brand spanking new copy of the form.
