If the road is mapped as a single carriageway and the distance marker is clearly visible from both sides of the road, then it would be simplest to make it a node on the way itself. Otherwise, it should probably be a separate node on one side or the other.
Of course the length of a road and the distance along it can be calculated from the OSM data and do not need special nodes every 1km or so.
