If the road is mapped as a single carriageway and the distance marker is clearly
visible from both sides of the road, then it would be simplest to make it a node
on the way itself.  Otherwise, it should probably be a separate node on one side
or the other.

Of course the length of a road and the distance along it can be calculated
from the OSM data and do not need special nodes every 1km or so.  But if the
object exists in the real world, it can be mapped.
