Hi Bryce, I don't think people read the text above OSM's notes submission form anyway. I fall in to the camp of "less is better". Mapbox blogged about how they do it recently. I really like the big "describe what's wrong" header as it achieves so much in so few words (and I'm sure if it made sense without it mapbox would drop the word "what's" too :-) ).
