Definitely see a Dr.- I was working on my bike a couple of weeks ago and jabbed a sharp pick into my right index finger- entry was just below the first knucke and the thing went all the way down into the webbing between the fingers. The next day it was very swollen and sore as hell. I went to the Dr. and he said they treat such wounds on the hand very aggressively because of all the tendons that run up the arm- it can get infected and travel up into the arm very fast and require surgery. They gave me a tetanus shot, a shot of keflex and I had to take 1500 mg of keflex for 7 days. It's fine now.
I also lost the end of my left pinky 30 years ago after shutting it in a car door. They grafted skin from my forearm onto the end of it, but it's about 3/16" shorter than my left pinky. I can still use it, though there are certain things I can't do, such as hold the pinky on the G strings as a drone while fretting other notes with the other fingers.
