FWIW, it's not clear whether protection might not be asymmetric, e.g., the server-side port is protected but not the client-side. The list also omits the IP pseuduoheader fields, e.g., IP source and destination addresses (the other fields, Reserved and Protocol, don't seem to matter whether protected or not AFAICT). For similar reasons, IMO it should be OK to protect the server IP address.
