As you no doubt know, most general research methods texts will cover these topics. Babbie's The Practice of Social Research (Wadsworth), Schutt's Investigating the Social World would (Pine Forge) or Neuman's Social Research Methods: Quantitative and Qualitative Approaches (Allyn & Bacon) provide a general overview of each topic.
I've found Bruce Berg's Qualitative Research Methods for the Social Sciences (Allyn & Bacon) a useful guide to observational research, in-depth interviews and focus groups. He's big on details, which I found useful. Cook & Campbell's Quasi-Experimentation: Design & Analysis Issues for Field Settings (Houghton Mifflin) goes into more detail than Campbell and Stanely, but is IMHO, more readable. (And how many other books written in 1989 are still in print....) There's lots of stuff out there on writing questionnaires, but Mildred Patten's Questionnaire Research: A Practical Guide (Pyrczak Publishing) is a nice, compact, "here how to do this kind of question" guide. And it's dirt cheap.
