I am at home so I have limited information but know of at least two sources. First, Carla wrote a chapter in Huber's and Morraele's (maybe 2002?) book on Disciplinary styles in scholarship of teaching and learning... That chapter contains a summary of some of this history. Second, Carla and I have a chapter in press for the 21st century handbook that also has a section on this history. Unfortunately, I don't have it at home and am headed out of town. But, I could send it to you next week if you want it.
