Everybody's different.  Here's my story:

I've used and enjoyed the Dvorak layout for many years.  But in terms of
wrist pain and such I got much more benefit out of using Kinesis
keyboards and foot pedals.  My 3 foot pedals are for Control, Shift and
Alt.  My keyboard looks like this:

The foot pedals saved my pinkies.  Having many keys under the thumbs is
great.  Plus the straight-instead-of-crooked reaches to move between
rows feels better.
