Hi Richard.  While I agree with you in principal, experience has taught me 
that large organisations can fail on the necessary discipline and small 
organisations have a diffcult time finding the resources to achieve what 
you are suggesting.  I only wish more organisations showed be a more 
disciplined approach to system management.   I do believe the continuing 
maturity of our profession is improving the situation though.
