I am slightly confused by the reference to insert key, do you mean enter? Anyway I have noticed that some versions of NVDA struggle to focus on the correct link on some web pages on some browsers.
If I ever encounter this problem I normally resolve it by activating the link not with the enter key or spacebar but by using Insert F7 to bring up the elements lists. I find the link or button I want to execute from there and pressing enter will in my experience activate the link 100% of the time.
