This was a bugfix to fix the the tests - no new features have been added
and no new code has been added to the main module itself.
The changes were:
1. Deal with the fact that the 'html' filter no longer turns '"' into
entitites (when did this happen?)
2. Be more flexible with the output of XML::LibXML wrt to <?xml and
<!DOCTYPE headers
