In response to recent list discussions, I've changed the default accidental placements when there are exactly 3 accidentals in a chord, and when there are no 2nds between accidentals. In this case only, they will be placed in the order top, bottom, middle. If there are 2nds, or if there are some other number of accidentals in a chord besides 3, then the old rules are still used as the default: starting from the top and working down, each one is placed as far to the right as possible. I also squished one more bug--a very subtle one--in the accidental placement algorithms.
