I'm ready and willing to make some updates in PMX on this. But I'd like some
input from the users. 

The only parameters I plan to have PMX put in the TeX file are \hsize,
\vsize, \hoffset, and \voffset. As it now stands, you can use Acl or Ac4,
which assign some built in values to all 4 parameters for letter or a4 size
paper respectively, and center the printed page within the paper size. Apart
from that, with commands h[n][u] and w[n][u] you can also separately set the
printed page width and height to whatever you want. 

I'd like ask both PMX and raw MusiXTeX users to report what they routinely
do to control printed page size, paper size, and centering, including your
favorite numerical values. Of course any other comments or thoughts would be
welcomed.

I'm planning on allowing PMX users to set values for any or all of the
following: printed page width and/or height, paper width and/or height,
\hoffset and/or \voffset, and whether or not you want it centered vertically
and/or horizontally. The puzzle is going to be working out a consistent
scheme for filling in the values of any of the 4 basic parameters that are
not determined by the combination of inputs a user chooses. The scheme must
cover all possibilities, ranging from no inputs at all, to too many (for
example, trying to set page width, \hoffset, and \hsize).

I'll use whatever feedback I get to decide about the defaults (i.e., if
there are no inputs), but I'll probably go with the existing \hsize and
\vsize, centered on letter size paper.
