BEING  A COMPILATIONTHERE MAY  BE ERRORS

Q1         Whatare some traits of genuine doctors?

A1         Tiny Nair, Cardiologist at PRSHospital, Trivandrum Updated 5y

Check for these signals

1. Deaf Doctor – despite high end scanners andbiomarkers, the most important
clue that clinches a diagnosis and decidesprognosis is medical history. If your
doctor is not focusing on your complaints (Deaf to yourproblems) and is
fiddling with his WhatsApp instead, it is better tostart your search engine all
over again.

2. Overconfident – Assuming that you have notgone with a viral fever or running
nose, any complicated medical problem needscareful analysis to make a
diagnosis. Just after listening to your problem with a cursory glance at your
medical record, this doctorsays, this is nothing, I shall cure you 100 percent,
Chance are high that youare meeting an unethical doctor. We never promise a
cure to anyone, we can’t.

3. Go to google - Not to diagnose your illness; butto check for the doctor’s
publications – so that you know his academicstandings (pubmed or research
gate). Remember, a good doctor also has a good standing among his peers,
shownby his publications.

4. Too Many or Too Few Tests - Most complicatedmedical illnesses need tests,
and your doc is likely to order them. If he is not ordering at all
orover-ordering, be careful. You go to your doctor with a
intermittentoccasional chest pain, your doc is likely to ask for anECG. If he
doesn’t or if he directly asks for a high end test like angiogram, add a pinch
of salt before you gulp it.

5. Check his medical degrees, this is true especiallyin India. In the US, all
doctors are MD, so not an issue. But in India medical degreescome from A to Z,
with many just worthless. (eg. FICA – fellow of IndianCollege of Angiology, a
degree carried by many Indian GPs, who don’t even knowthe expansion of these
letters). You can buy them outright. No Good doctor willever flaunt a worthless
degree. The standard degrees are MBBS, MD, DM, MS, MCh,DNBE, (all Indian) and
MRCP, FRCP, FRCS (UK), BE / BC FACC etc (US), EuropeanBoard Certification (FESC
etc.) the list continues.

6. A patient might appreciate a doctor simplybecause he makes him feel better,
which is by no means a small thing. We do want to feel better afterconsulting
our doctor. But if you wish to know about his academicexcellence, you either
have to search internet or ask other doctors about him.

7. Quality is in Details – Most senior doctors willpersonally take a detailed
history and a thorough examination before arriving at a diagnosis. Becareful if
he is not.

8. A good doctor would finally tell you all that hehas made up of your illness,
the treatment plan, the outcomes and also the uncertainties. A doctor who is
not ready to do thatand says “eat this and you would be alright” is not alright.

9. A good one always seeks second opinion oncomplicated problems, we do it so
often. Refer to a colleague and ask ifsomething else lights up in his mind.
This only shows that the doctor is worried about findingsolution to your
problem, not thrusting down his ego down your throat.

What about Top 10 List?

Once I wastravelling with senior American cardiologist, (holding a senior
position in theAmerican College of Cardiology; ACC) in a transatlantic flight.
The inflightmagazine showed this list of “Americas top cardiologist”.
Surprisingly, itturned out that this very senior ‘academic’ cardiologist in the
US has Neverheard the names of any of those “TOP 10” cardiologists listed in
thatmagazine!!!

Todaymoney can buy anything, both in India and US; Name, fame and
photoshootinclusive.

So choosing a good doctor is not an easy task.

Edit 1. — The Diplomas I missed out (D ORTHO,DGO, DLO…… ETC) thanks Runak.

Q2         Whatare some best tips and tricks for people visiting India for the
first time?

A2         Intekhab Alam, Avid traveller Updated6h

Never travel in aSleeper class.

Back in 2016, I was travelling to Rajasthan in thesleeper class. A young couple
from France boarded the train and sat right nextto me. After conversing for a
while, I asked them why did they take the train and why choose the sleeperclass?

She said she wantedto experience the “real India". Fast forward 3hours, they
were irritated beyond limits. The toilets were unclean, people keptbothering
them and to top it all, they were sweating profusely.

If you plan on using the train, book second class ACtickets. It won't take away
the Indian experience that you're seeking.

Always check thewater supply before using public toilets

It goes withoutsaying that you should probably avoid using the public toilets.
However, if youcan't keep it in, make sure to check whether the toilet has an
active watersupply. Word of warning, you won't find toilet papers anywhere. So,
good luckwith the public toilets.

Lodging

Do notcompromise with lodging. If you're travelling, you willfind plenty of
cheap places to crash for the night. If you're using them, do athorough
background check. Check whether the toilets have hidden cameras or not (itis
more common than you can imagine). Pretty sure you wouldn't want to regretyour
stay over a few bucks.

Bargaining skills

If you're aforeigner visiting India, there's a 100% you will get swindled by
the local vendors. Theyask for 10x more than the actual price. If you want to
hold on to your budget, make sure you have your bargaining skills refined.

Street food

Go easy on the street food. Do not overeat.Always carry your own water bottle.
You'd definitely not want to spend your daycoming in and out of the toilet.

Apart from theabove, India is THE place to travel. The people here are super
friendly and attimes you may find that creepy. However, remember most of them
do not mean you any harm. In fact, youwill across families who will invite you
over for lunch/dinner.Have a safetrip!

Q3         Howis blood-cell plasma formed?

A3         Ken Saladin, Former professor ofhistology (microscopic anatomy)Aug 9

It’ssimply called blood plasma, not blood-cell plasma.

Blood plasma

Most of the plasma is water, which is absorbed into thebloodstream from the
intestines and continually exchanged back and forth withthe fluid in one’s
tissues.

Next to water, themost abundant component by weight is protein—albumin,
fibrinogen, and a fewdifferent kinds of globulin. One of these, gamma-globulin,
is made by certainimmune cells that develop from the B lymphocyte type of blood
cell. All the rest of the plasmaprotein is made by the liver.

Mostof the plasma components listed under nutrients and electrolytes below
comefrom the diet, absorbed from the intestines. However, theliver makes the
lipoproteins, excretes much of the cholesterol into the blood, and both provides
and removes (in a balanced exchange) glucose, iron, and aminoacids.

The lactate andnitrogenous wastes are products of body metabolism that the
blood acquires frommany different organs, and the gases, of course are
exchanged between blood andair in the lungs.

Q4         Doesunplugging a TV when not in use make it last longer?

A4         Gopala Krishnan,Former AssistantGeneral Manager 1996-2004 at
Department of Telecom (1966–2004)Just now

I am writing fromChennai, India. I am having MI TV in hall and old LCD Samsung
TV in my room. Afew months back one guest came told me, his MI TV motherboard
failed inlightening during a rain , and desired to have stabilizer . Next day
itself I Installed astabilizer for it.

TV should not be continiously used.It requires rest. It is best TV is switched
OFF for about an hour after 5–6hours of working.

Present day plugpoints are controlled by switch. Most control TV using remote
for switching ON and OFF. Manytimes TV may have plug point at back of the TV.

It is best ifyou are leaving home for months to remove the TV plug from socket.
Normally ifstabilizer is provided, in a sudden lightening stabilizer only will
be affectedand not TV. If it is winterand raining, if switch in the plug is
convenient, you can switch OFF TV fromswitch rather than from remote.

My point-A stabilizer may protect TV from surges during lightening.Best switch
OFF TV from remote rather than from switch, during a lightening period. You
willbe protected by switching OFF from remote.

Q5         Isit good to sleep during the afternoon one or two hours daily?

A5         Lois L. Kersey, Aug 2

We have all taken anap at some point. Several people do it daily, but little is
known if theyprovide health benefits or, on the contrary, harm it. That is why
Dr. EvelynBenavides, adult neurologist, specialist in sleep medicine, clarifies
andexplains the pros and cons of taking a nap:

Advantages:

Naps are favorableas long as they do not last too long, since the ideal is that
do not exceed 40 minutes andthat are not after 4:00 p.m. For preschool
children, a post-lunch nap isrecommended if they need one. Some people have the
habit of napping to helpthem perform in their daily activities. The important
thing is that you do notexceed the recommended schedule and time.

Disadvantages:

In people who suffer from sleep disorders, such asinsomnia, the act of napping
causes the few hours they sleep to be spread outduring the day, which can
increase insomnia.

In olderadults it is not so recommended, since they have a lower need for sleep
thanyounger people, reducing the duration of nightsleep, which is the one that
provides the most benefits.

ABSTRACT:

For a nap togenerate benefits, it should not last more than 40 minutes or sleep
after 4:00p.m. Children have no problem doing it after lunch, while it is not
recommended for older adults.

My note- Older adults get siesta  after meals in thafternoon. If at all any
work is there, they are not able to concentrate anddo. Result is they sleep
less in night and get up early.

Q6         Asa person who lives abroad, what is difficult about going back to
your homecountry?

A6         Bhuvana Rameshwar, married for three decadesUpdated Sun

I lived abroad fora decade almost as our father worked in Nairobi University
(Kenya) inengineering dept. My brothers stayed more years as I came to India
for myhigher studies. We visited India once a year during our academic
vacation.Sometimes once in two years.

Each year we felt we missed out more and more onrelationships, the fun, the
festivals the celebrations. We felt out of place as our Tamil had changed, our
English was different andthe people in our grandparents house and family
treated us like outsiders. Ourrooms, furniture, our space taken away. Years
later we losteven our rightful share in our grandparents property and jewels.

We just couldn't goback to the old times again. However much we tried it was
always like we wereleft out of the fun. Any number of gifts, outings andget
togethers couldn't bring us all to the same wavelength.

Slowly the fun ofair travel lasted only till we reached our native town. The
same airport thesun, the cab, the roads sides with big cinema banners, the same
main roads andstreets, the corner temple gave us anxiety. Butterflies in the
stomach werecommon in us. We weren'thappy anymore to see our extended families.
We were happy to be in India, andTamilnadu but not in going home. There seemed
an invisible line betweenus and our grandparents too.

What made it worsewas the seemingly simple but most irritating question put
forth by our uncles’wives “how many days are you all staying? ” or when are you
allgoing back?“ … as soon as we entered the same old, nostalgic grand
parents’house. It was clear we didn't belong there.

Hmmm…

Q7         Canan electrician install a ceiling fan without a fixture?

A7         Gopala Krishnan,Former AssistantGeneral Manager 1996-2004 at
Department of Telecom (1966–2004)Just now

You mean a U bendfixture in RCC roof which is internally put on steel bars with
a bend to keepin position? Or a strong hook which can be connected to a beam in
a tiled home?A mason will help to break open the RCC and find steel roads to
connect hook.He will plaster and after drying ceiling fan can be mounted. A
fixture to awooden beam can be fixed by electrician himself.

A ceiling fan canbe installed only with a fixture to connect the fan rod.

My note- I did not want to add this inthe Quora  answer. For reasons unknown,
the new telephone exchange building at Vellore finished civil works  without
any provision for fans. Whereever fans were required RCC was broken, fan hooks
provided, Re-plastered andafter drying fans were provided.

Q8         Wheredo trains go when not in use?

A8         Aditya Agarwal, Lives in New Delhi(2015–present)Updated Jul 22

Your question has two meanings. First is you mean thatafter reaching its
destination, where does a train go and second is when lifeof a train is
complete and it cannot be used as a passenger train what happenswith it.

I would give answerfor both questions:

For first one, after reaching its destination train goesto rail yard, where it
gets washed and cleaned before starting its next journey.Water is filled in
water tanks of all coaches. Many technical things arechecked in all coaches
using different devices.

Water is filled inwater tank in coaches.

Forsecond one, when train cannot be used as passenger train, Indian railways
use it as parcel coach, freight carrier, repair workers coach.

Passenger trainconverted to automobile carrier by closing windows using metal
sheet.Oldpassenger coach converted to new goods carrier.

My note- Shorttravel passenger trains may have return trip within one hour.
Inthat case train doesnot goes to the yard. Cleaning and water filling is done
in the plat formitself. Eg- Brindavan express, double decker etc from Chennai,
return onthe same day after an hour from Bangalore. Similarly Bangalore Chennai
expressreturn after an hour as Lalbag express from Chennai. After that only
they go tothe yard at Chennai/Bangalore for next day trip.

Q9         Howdo people in Sweden (and overall cold climate countries)
ventilate theirhouses? Do they open the windows?

A9         Chris Ebbert, Lives in Sweden Jul 23

Opening a window would be the standard approach forventilation, yes. And the
northern European classicof window mechanisms is this, the inward-leaning
window opening for ventilation(the Germans call it “Kippfenster”):

You can see it inuse in this image, where the very left window is opened in
this way.

I have not seenthis used much elsewhere. You see it pretty much everywhere in
Germany,Austria, Sweden, France, and so on, but I have never come across it in
Canada, the US, China, India, the UK, or New Zealand.

It has a good number of advantages, such as:

it’s less visually obvious    if it starts to rain, no problem    birds and
rodents are less likely toenter    wind won’t bang it around orslam it shut
no one can just enterthrough it, making this an option that feels safe at night.

People even leave these open as shown while going to work. There are burglar
proof versions of these, and theyactually are quite burglar proof just by
default.

Other than that, houses are built to “breathe” to a certain extent, of course.
There is a natural attrition of airat all times, through door seals and window
seals. Cat doors areactually an enormous ventilation gate, I find, to the point
where I’ve foundmyself compelled to build an air lock for our cat door.

You probably won’tfind this anywhere else.

Many houses actually have ventilation ducts for passiveventilation built in,
not unlike cars. Look closelyat the gables of some houses, and they may have a
little grid there. The thingis of course that ventilation must continue even
when it is so cold outsidethat you can’t open any windows.

Q10              Whatwill happen to the permanent jobs of Indian Railways
employees afterprivatization? Will they all take VRS?

A10       Kanthaswamy Balasubramaniam, Advisor andTutor at Staff Selection
Commission Combined Graduate Level Exam(2021–present)1y

They won’t Privatise Railways

The Railways is toomuch of a Crown Jewel to be Privatized

They may Privatize the Management of Stations, Ticketing,Booking, Food etc and
They may launch ExclusivePrivate Coaches and Trains but The Railways has too
much infrastructure to beprivatized.

Yes it will be slimmed down. This is already happening. In1990 - Indian
Railways was 43% over staffed, in 2008 it wasonly 26% Over Staffed and now
in2018 it is only 9% over staffed. The simple strategy is to hire fewerpeople
and distribute more work among the rest.

It would take anAbsolute Idiot to Privatize the Railways completely
