After your explanations I resume for me, I modify the size of the ready map in or with the program which I use for making my publication. That I have also done in the past. I have thought there could/must be a possibility directly in Therion. What a shame.. If you export map -f pdf, then you can indirectly influence the page size by adjusting the scale and where you place the header. Not as precise as you are wanting. If you export atlas -f pdf, then you can precisely specify the page size, as you have been attempting. The cave is then distributed across as many pages as is required.
