Thanks for the inputs! I ended up disconnecting the TX and powering up the windings separately to figure out the wiring.
The wire on pin 8 did indeed to turn out to be mis-wired! The added taps allowed for 120/240 or 100/200VAC inputs. Got it wired back up and it's working fine at 120VAC now!
