Are the MAX3232's genuine? I've had those issues with counterfeit ones but
the genuine Maxim ones have been rock solid for me.

Also, are the caps correctly sized? The advertised requirement for 0.1uF
caps is only for a particular voltage that I don't recall. Different
voltages in the range of acceptable voltages require different
capacitances; check the data sheet.
