 * check if other servers have that spike, too.  If so: nothing to worry 
about, just a glitch in the network to the monitoring system (the 
monitoring scripts themselves are, obviously, bug free.)

 * if you write log files of ntp, you can check if ntp itself did odd stuff 
(jumping around, changing primary time source) and if it did think it had 
an offset spike.

 * check your own network, as Peter said.

 * don't worry too much, as there's nothing you can do 99% of the time.
