Nothing, if you have done what you have said.

Most firewalls will let you log rules, so perhaps you should put logging 
against the port forward to see if the connection attempt is getting through - 
your ISP may be dropping the connection.

If the server is linux you could use tcpdump to confirm the connection is 
making it all the way through, and check the routing to make sure the reverse 
path is there (just make sure it has a default route pointing at the same 
firewall).
