Here is $.02. 

It seems to me that the issue is what institutional affiliation 
that you will use.  If you list your original institution then I 
would list both you and the ex-student as being at that 
institution.  The authorship affiliation would indicate that the 
project began at that institution for both of you.

On the other hand, if you list your current affiliation then I 
would list the city and state where your ex-student is currently 
located.  See page 296 of the APA manual.
