What are your .qmail files?
You should have:
.qmail-acces (which runs tmda) .qmail-acces-default (which is a symlink to .qmail-acces)
Or if your username is 'acces', it should be:
.qmail (runs tmda) .qmail-default (symlink to .qmail)
