I suppose the next things to check are:
LOGFILE_INCOMING - what does it say about the incoming messages?
The contents of the file ~/.tmda/lists/confirmed - Are the confirmed addresses being added?
What happens if you explicitly add an address to the whitelist file? Does it get challenged too?
