I have seen this also with my server. The only way that I was able to fix this was to create custom templates for each user. Basicly copied over the global confirm_request.txt and confirm_accept.txt to %HOME%/.tmda/templates and altered the headers of each template. There you can also change what the confirm_*.txt says also to make it more personal. For me the global templates are in /usr/share/tmda
