I just tested it with the previous version (1.0.2) but I still get a second mail to confirm:

Why does TMDA does not check that this mail is a confirmation mail? Are there no header infos needed? Only the mailaddress with +confirm+xxxx.xxx.xxx is checked?
You do have both your MTA (exim/postfix/sendmail/qmail) and TMDA configured for the correct "recipient delimiter"? In your case, TMDA obviously thinks it's a + sign, since it generates confirmation addresses with that as the delimiter. Perhaps your MTA is configured to use a - sign, and hence the extensions aren't being correctly indicated to TMDA?
