I downloaded the TMDA Tools plugin for SquirrelMail and had some issues as well. The main issue I found was the permissions problem. What I did was to make sure that apache was in the same group as my MTA, which in my case was vpopmail. Then I changed the permissions on the pending directories to 770. That solved the issue for me.
Incidently, I rewrote some of the plugin to be able to manage the lists in a MySQL database as well as added a few other things that I saw as neccessary. The reason being is that we're wanting a single location for handling our lists across multiple applications.
