To me, this looks like a qmail installation problem, and nothing related
to TMDA.

Can you send email to mbox.fizbin.com addresses yourself (i.e. from your
mail client, just like any other mail). I *guessing* you'll see the same
kind of issue.

If not, please check your qmail log files and see if anything else
useful shows up regarding that domain.

Also, you can try checking the TMDA log files, which are typically:

~/.tmda/logs/debug
~/.tmda/logs/incoming
~/.tmda/logs/outgoing

although these can be changed via config file settings.
