You need to find something like
Received: by simscan 1.1.0 ppid: 16302, pid: 16308, t: 3.3615s
         scanners:none

You got something like
:allow,QMAILQUEUE="/var/qmail/bin/simscan"
in your tcp.smtp?
and the cdb uptodate?
