I don't (personally) have access to an IIS server to continue investigating. If it was on one of my systems, the first thing I would try is to write a simple ASP page that does the same thing, to see if the problem is with IIS's ability to parse headers. If the ASP page works as expected, then you may have to get the source, and add extra logging statements to resolve it.
