Load balancer broken in JK2 with IIS

Using the load balancer in JK2 with IIS produces the following errors in 
the event viewer and appears to hang for each request:
Error: [jk_isapi_plugin.c (496)]: HttpExtensionProc worker is NULL

Anyone have any ideas?
