I've been using http://www.kgbinternet.com for about 3 months now and they're awesome! You get your own Tomcat instance and you're allowed to upgrade at will - comes with JDK 1.4 and one-time setup fees for MySQL or PostgreSQL. Seems like a small shop and Keith has always been quick to respond (within 1/2 hour via e-mail) of any issues I have - and only $12 US/month!
